Purpose — what it's actually for, so you can ask informed questions

Prescribing provider — which doctor ordered it, in case something needs to be confirmed or changed

This isn't a list you fill out once and forget. It's meant to travel with you — to appointments, to the pharmacy, into the hands of a sibling or backup caregiver who needs to step in. This was built from our family's experience.

During our mother's dementia care, medications changed often — a new specialist here, a dosage adjustment there — and more than once we found ourselves trying to reconstruct the full picture from memory in an exam room. It was avoidable, and it was stressful in a moment that didn't need more stress.

This template is the tool we built so that never had to happen again.
