You don't need every field filled in to make this useful.
Start with what you know off the top of your head — the medications you're most familiar with — then fill in the rest as you confirm details at the next pharmacy pickup or appointment. A partially complete list is still more reliable than trying to hold it all in your memory.
Once it's filled in, keep a copy in your bag or car, and consider taping one to the inside of a kitchen cabinet or pill organizer lid — somewhere it'll actually be seen and used, not filed away.
