Fill it in completely, then put it somewhere obvious.
Not filed away in a binder. Somewhere it will actually be found in a hurry — taped inside a kitchen cabinet door, tucked with the medication organizer, or kept in the front pocket of a medical folder. Consider making two or three copies: one for the home, one for your own bag, and one for any sibling or backup caregiver who might need to step in.
Update it any time a medication, provider, or insurance plan changes. A sheet that's six months out of date is better than no sheet — but a current one is the goal.
